Attachment to w post
Its an E. Hartford High Standard, low mileage, and I used CCI Std Vel. Fired case is still in the extractor hook after the slide partially picked up another round from the magazine. The newly striped ctg is heel down, nose fwd low contact on the face of barrel and the previously fired cased still being held by the rxtracto.
I have a photo of that but do not know how to post an attachment.
I'm thinking the cause most likely due to powder puff load but thats kinda rare.
I don't know the cause(s) but I'd appreciate reading your thoughts on whst was the probable cause and how to fix it.

Thanks for the reply. Shortly after I made that comment about ignorance in posting procefures, I saw the instructions that I needed to know.
What I did not do was post the issue about failure to feed issue in my low mileage High Standard. Now that I know how to post images as well as words, I will ask about feed issues and have photos to help explain the feed gotchas.
